Health and Consent Form

Grandview requires a health and consent form children who participate in the Children’s and Youth Ministry. Please complete and submit this form to the youth or children’s minister before your children attend any activity off church grounds. Completion of this form digitally online is preferable.

Please put the name of the child/youth in the “Your Name” field. If this pre-fills as the adults name, please open the original form link in a web browser instead of through the Church Center app.
